Nope. The closest was Super Bowl IX (2-0 Steelers at the half, and they scored with 7:49 left in the 2nd quarter).

That may be, but the Panthers have only 1 challenge left (they burned one on that bad spot that was upheld).
1st and goal at the 5. I'd pay to see the Pats come away empty this time (though I doubt that Vinateri would come out on 4th down this time).
