The area of arousal is in the front. When you normally do a pelvic exam, you stretch the back to get in the speculum. Except to touch the side to "warn" the lady, you aren't supposed to touch/caress/stimulate any area on the outside...on the inside, it's pretty numb...

Except for a few inches inside and along the frontal wall. Touching that area creates pelvic thrusts in a little while. But a NORMAL doctor wouldn't do that.
