I've been thinking about the pictures of Terri, sitting up in her chair, etc., and the close-ups of her eyes following her mother.
Am I wrong or does there appear to be a "hole" at the base of her throat? Wouldn't that indicate a tracheotomy, a procedure I don't recall being used with heart attacks.

Yes. Perhaps when they first got back into a normal breathing pattern, the operation was necessary-common and life saving.
Do you think Michael lost control and beat her, causing this injury?

They do trachs if someone cannot breathe through the mouth or nose. It's a more direct route to the lungs. They do that if someone is found at the bottom of a swimming pool all the time in Florida. Trachs are a common procedure, sometimes for heart patients and stroke patients.
