If you really want to see heavy smoking, visit mainland China. The PRC government has a stake in the sale of tobacco.

IIRC, Japan Airlines is one of the last remaining carriers that allows smoking on many of its flights. Not to the US, of course, but on intra-Asian and domestic routes.
