A person who is 500 pounds, probably realizes that what they are is not ideal, even if they feel they can’t help themselves.

In their hearts, most of them know they are sinning. That's one of the reasons they're working so hard to get society's approval.
I don't recall any cities being destroyed for gluttony, like Soddom and Gomorrah.
