When Jesus said, "if he will not listen to the church, treat him as a pagan or tax collector," what did He mean? Well, since Rome was at that time the tax collector and it's state religion was pagan, it seems we can rule that out.

(ekklésia): but if he neglect to hear the church, let him be unto thee as an heathen man and a publican.

Short Definition: an assembly, congregation, church
Take it to the congregation or assembly of believers that he has been meeting with. There is no inferred worldwide hierarchy there.
