Here is a very uncommon style of icon of the Visitation from a church in Cyprus. Its from the 14th century and shows Christ in the womb of the Theotokos and +John the Forerunner in +Elizabeth's. Notice how Christ is blessing His cousin and +John is bowing:
Theotokos Virgin, rejoice, Mary full of grace, the Lord is with you. Blessed are you among women, and blessed is the fruit of your womb, for you have borne the Saviour of our souls.
