Actually, there was a complaint filed with the FLA DCF. This is from the petition to Gov. Jeb Bush:
We also request that you review the -heavily guarded- Department of Children and Families (DCF) sealed investigation report. It contains the agencys findings in response to a formal complaint filed of alleged abuses Terri suffered at the hands of her husband and guardian, Michael Schiavo.
I know that I also read something about this in one of the articles about Terri. As I recall it was a rather lengthy report, but the DCF decided there was no reason to take action. I can't seem to find the article now, but I remember when I read it it said how many pages long it was, and I thought it was a LOT of pages for there to be NOTHING worth investigating there.
Of course, seems to me the FLA DCF has been in the news quite a bit over the last few years with all kinds of screw-ups and problems, haven't they?
The DCF was asked to investigate possible abuse of Terri prior to her collapse. I'm referring to any reports/requests addressing Michael's abuse of his WARD, since he's been appointed her guardian and has neglected her care and rehabilitation.
That DCF investigation report really is sealed from prying eyes. Nothing for Terri on the state site. But more searching, an article by WorldNetDaily came up, dated August 2, 2003.

The person to contact with complaints about Judge Greer, is, I believe, the State of Florida Courts Administrator, J. William Lockhart, who forwards valid ones to Demers. I think he is located in Tallahassee.
So far, though, all I've found is the address of the Supreme Court of Florida, 500 South Duval Street, Tallahassee, Florida 32399.
