There's a photo of Bubba smoking a cigar, but he claims it wasn't lit. (The smoke is apparently a figment of my imagination.)

Did they have to smoke in the White House? Or does smoking in the high school bathroom when they were 17 count?

The Last President to smoke in the White House (cigarettes not doobies) was probably Gerald Ford. If you count doobies, it was probably Clinton.

The last President to smoke in the high school bathroom was definitely GWB.

Anyway, let me take a guess on your questions. Dwight Eisenhower. Though I know some of the First Ladies smoked cigarettes too including Nancy Reagan and Betty Ford.

When I was a Midshipman I had volunteered to spend some time at Quantico. This was during Nixon's administration.

While there we were taken to see Marine 1, the President's helicopter. In the hanger there was also Marine 2, a UH-1 painted the same as Marine 1. The hatch was open and inside the door I could see a small bracket which was holding two packs of cigarettes (Lucky Strike filters, I believe). So my guess is Richard Nixon.
