Agreed. Smallpox (or any communicable disease) would quickly become a global epidemic, and it would hit most effectively in third-world countries (e.g., the Middle East, South Asia, Indonesia).

Yes, but the thought does occur to me that we, as rational humans, quickly see the fallacy of terrorists releasing smallpox into the world. The terrorists, being madmen with an insane agenda, might see the death of millions (including Muslims) as a means of moving Islam's clash with the West forward.
