It’s a wonder that any of these large (nationwide) comms systems don’t have more outages. If the public knew how easy it is for something fairly simple to go wrong and take out the whole shebang, they’d revolt.

In properly designed systems, only approved changes can be moved to production. Without a valid, scheduled, approved change, you can’t draw the password. Normally, ordinary programmers have no access to production whatsoever, as all changes have to be implemented by production control team, whose instructions are that all required procedures must be followed no matter what.
This does save a lot of trouble.
We exist on the edge in all infrastructure systems. Thats why the people in charge have high stress jobs. It gave me heart disease, diabetes and TIAs, and I retired as soon as I could, that job was killing me. Ive known many other management types who have died in the saddle.
