Idiots run TV stations. How about the time an Asian airliner crashed at San Francisco Int'l Airport, and KTVU-2 scooped everyone with the names of the flight crew. Something like this.

"Captain Sum Ting Wong, Captain Wee Tu Lowe, Ho Lee Fuk, and Bam Ding Ow"

Got past all the editors, and the female anchor announcing it.

Portland, Oregon NBC affiliate KGW ran a viewer-submitted photo prominently featuring the n-word during the "Throwback Thursday" portion of their "The Good Stuff" good news segment. It can be seen at 0:21 below. They've since spent a lot of time apologizing.
