Racial, ethnic, and gender diversity lacking in surgical leadership

Gender, ethnic, and racial diversity are lacking in leadership positions across U.S. academic surgery departments, according to a study published online Oct. 11 in JAMA Surgery.

Yoshiko Iwai, from University of North Carolina at Chapel Hill School of Medicine, and colleagues assessed how racial, ethnic, and gender diversity among U.S. academic surgery department leaders differs across roles and surgical specialties.

The authors searched publicly accessible medical school and affiliated hospital websites in the United States and Puerto Rico (154 surgical departments affiliated with 146 medical schools; Jan. 15 to July 15, 2022). In total, 2,165 faculty were in included in the analysis.

The researchers found that there were more men than women in leadership positions at all levels—chairs (85.9 versus 14.1%), vice chairs (68.4 versus 31.6t), and division chiefs (87.1 versus 12.9%). Furthermore, only 192 leaders (8.9%) were from racial or ethnic groups that are underrepresented in medicine (URiM).

Both overall and within racial and ethnic groups, women occupied more vice chair than chair or division chief positions. URiM individuals were most commonly seen as vice chairs of diversity, equity, and inclusion (51.6%) or faculty development (17.9%). URiM representation was greatest for division chief roles in transplant surgery (13.8%) and lowest in oral and maxillofacial surgery (5.0%).

Just what I always wanted, my surgeon selected on racial, ethnic, or gender basis rather than competency. We’ve seen what diversity has brought us as a Vice President.

There are several major reasons for the lack of diversity. Ability, competence and desire to work hard. Quite honestly, I don’t want affirmative action doctors working on me and I certainly don’t want a doctor who cares about diversity. Caring about diversity shows an extreme lack of intelligence and judgment. Those two qualities you never want to see in a doctor.

If I go into any, ANY professional situation and a tranny is there I will leave. I will have nothing to do with those freaks. Not for one minute in ANY situation.

Every so often, I need surgery for an ongoing situation. Due to damaged nerves in feet and legs. Usually, same day surgery.

The End Game is like this: I do not want my next Surgeon or even my next Anesthetician coming up to my bed to introduce themselves, while they are cross dressing, or wearing long furry Dreds, or with a face full of iron piercings, or with that 3 line Indian chin tattoo.

The surgeon is not supposed to be a visual showboat or an unnecessary distraction to the patient or to the team of doctors. Everyone knows the standard appearance expected. Get with the program!

Personally, I’m waiting for all those Cracka pilots and air traffic controllers to be replaced with a whole crop of DEI/CRT/LGBTQ+/Affirmative Action folks.

Silly me. I have always thought that talent just might be a factor in surgical leadership. And ironically, we all know an African-American brain surgeon that’s world famous...but he is not a clueless left winger, so never mind. As for me, I choose talent. When I want fake and rather stupid, I’ll pick Dorkbama.

I have another perspective for the lack of URiM in leadership positions. My daughter is a cardiologist and trained at Emory University and worked at Grady alongside med students from Morehouse. I asked her why there were so few blacks in her field. She got to know some of the guys from Morehouse fairly well and offered this. Many of these young black men are supported by churches, friends, and local businesses throughout their medical training. Not through loans, but through small amounts of money sent to them for food and living expenses. They are encouraged and expected to get through medical school and residency and then return to their communities to serve there. They aren't thinking about higher positions - they are thinking about the immediate need.

Meanwhile those that don't come from that past and mindset are expected to reach as high as they can. They aren't being pulled back to their homes. In turn, this keep black doctors near their communities and out of top positions in academia and chairmanships.

Given an option, I’d always pick the male Asian or White, preferably Jewish. Heterosexual if you can figure it out.

Then, outside of Senator Fauxcahautis situations, you know it’s not an affirmation action idiot cutting you open.
