Maybe nukes have been miniaturized enough to bring components of a nuclear weapon into the USA in small pieces in the backpacks of gotaways. These components could be reassembled anywhere and then placed in high density population areas of large cities. Also, these gotaways could be carrying nuclearwaste from the muslim brotherhood of Iran in small amounts and and dumped anywhere on the streets of major cities. In a few weeks people will become ill and large patches of their hair will start falling out. The medical staff won’t know how to treat millions of people at once from nuclear poisoning.
