I think it is more posturing and saber rattling. Iran doesn’t have the ability to project power effectively. But they might be able to smuggle a nuclear weapon into the harbor at Haifa and blow up the port. If they did that Iran would become a parking lot.

The Iranians do realize if they nuke Israel Gaza is also destroyed. So in theory, Iran would kill over a million Palis to rid the Jews from Israel and no one will occupy the land for a millennia.

most likely explanation, even though it is a thing called War, the Israelis would be extremely careful NOT to hit a hospital ,
even hitting a vacant medical building used as a shelter for a Hezbollah missile launching site would be a propaganda gift to Iran
