Something changed in the Republican party? If so, I wasn't aware of it.

Cindy McCain says late husband John 'wouldn't recognize' today's Republican Party
Oh, I think McCain would recognize Mitt Romney, Mitch McConnell, Susan Collins and all of the usual double-dealing anti-conservative GOP members easily enough. The task is to actually make the Republican Party something McCain would not recognize.
As for Cindy McCain, she should simply start campaigning for Democrats and stop pretending she's anything else.

They hid McCain at the United States Naval Academy Cemetery so people wouldn’t visit him with a 12 pack of beer and a bag of burritos, leaving a “tribute”.

I wish she were right. But, there are still plenty of Republican establishment types like her late husband willing to lie to get elected and betray their base.

If she’s talking about GOP voters, then yeah, there’s been a big change, but still not big enough.
If she’s talking about GOP politicians, barely any change at all.
These Uniparty hacks can pretend that too many GOP politicians are beholden to the “Trump/Q Anon wing” of the party.
The truth is, they hate their own voters and always have. And now that many of us have had enough, they are lashing out at us, and falsely claiming we’ve been brainwashed with 🦇 disinformation 🦇

As usual, the Daily Mail only tells part of the story. John McCain began the hostilities between himself and Trump. In July 2015, Trump held a campaign rally in Phoenix, Arizona. McCain was upset because Trump didn’t kiss his hand, and get permission for it. McCain was interviewed about the rally a few days later. His quote is: “It’s very bad,” the Republican senator said. “This performance with our friend out in Phoenix is very hurtful to me,” McCain said. “Because what he did was he fired up the crazies.” Trump was asked about McCain’s comments a week later, and responded: ‘I like people who weren’t captured.’
