Sounds to me like it was his therapist (I’m guessing he already had one, or several from the sound of his guy) was dealing with him and he was not ordered by the court to see a therapist (no law enforcement action). The gun purchasing form is for court-ordered mental health treatment.

A prohibition based on mental health must be adjudicated by a court. Not a simple diagnosis of some mental issue or defect. And it must always be so. A legal determination that one ought not have certain liberties and exercise of rights. Otherwise we all will find ourselves prohibited by ....some arbitrary and capricious non legal act.

In this case, it does seem that Crimo ought to have been involuntarily committed and adjudicated by a court. But, unless his freedom of movement was restricted, he or anyone could obtain a weapon etc and act criminally.
