It is real. And Joe can't resist grabbing young girls. He isn't molesting her in the photograph, he's just holding her in a less modest way than Prince Andrew is holding Virginia Guiffre in the infamous photograph of him with Ghislaine Maxwell and the young girl reportedly trafficked by Epstein.
Most men don't touch young women or girls the way Joe Biden does.

In my humble opinion, Biden is not accustomed to having any supporters around him (does he have any who don’t work for him.) so that he was thrilled to be in this picture. His voters will never see it.
And I counter you by saying the quiet part out loud - Joe’s perverted craving for young girls (yes it’s sick for me to even be typing that but it’s observably true) - is greater than his hatred and anger for Trump. Sure I bet he doesn’t grasp the optics of how predominant the Trump kids are - but he doesn’t care.
Did you see the video of the guys booing Zhao Xiden at Ground Zero?? At one point Zhao makes his way towards a kid that’s standing and watching and the guys start yelling, “Don’t sniff him!! Don’t sniff him!!”
