If you have enough money there's very little cosmetic/plastic "surgeons" can't do these days. EXCEPT...nobody no matter how skilled they are can change basic DNA, the endocrine system (if he stops taking artificial hormones, the ruse can't be maintained), implant a functioning reproductive system or alter basic bone structure and density. There have always been effeminate men and manly-looking/butch women, but they are STILL men and women.
