perhaps it has something to do with general health levels of the long term residents; MS in particular is notorious for an overweight and sedentary population...

Maybe. Hawaii has to be because of the native Hawaiian population being a more susceptible host. They have been getting wiped out by European diseases ever since Captain Cook made landfall and they appear to be getting hit by Covid harder than most too.
