I dunno what it is, but it seems odd he’d have any ‘ear piece’ equipment there. An IV? Mayybeee, a pen would also be kinda weird, but more likely I think. People don’t walk around with an IV much, and if there’s no bottle over his head with fluids, that really makes no sense.

People who need meds daily/hourly have a shunt(?) to avoid constantly being poked. Commonly called a picc line.

Google “peripherally inserted central catheter (PICC)”. These are inserted for longer term use for things such as chemotherapy treatments, other medications, liquid nutrition.
