Not at all clear. It is hard to pin down causes. Many times the parents are to blame.

The kid would be better off learning to accept who HE is, and for now, more or less conforming to societies definition of male and female. If he still felt that way after high school, that would be his business, but at least he will have learned to accept the world as it is now, vs expecting the world to change for him.
The family is not helping by going along with the fantasy. That means everybody has to participate in the lie, that or be cast out of the family itself.
Wow. One moment of correct parenting could have saved this miserable future.
