How does one accomplish such a thing? Not that I care what she did - or really anything about her personally - but I’m intrigued with the logistics of banning that many accounts.

Hire some IT guys to do it. And for cheap.
There are lists you can buy of "undesirable" twitter accounts, but most likely she had a custom list made. Then you just upload the list to the blocking application.
Lots of stories out there of deplorables being blocked from looking at twitter accounts of people they've never interacted with before.
