tales of heroism, but still no coherent explanation of what happened. I can't imagine how a US combat vessel did not see and avoid a container ship.

Just to clarify...this could be an attempt by the Navy to officially make it a failure of leadership.
But either way, seems like a bigger problem than one ship colliding with another, which is big enough.

Facts gathered so far.
Captain was asleep.
Most of the crew was asleep.
Some of the crew ‘on watch’ or ‘just awake’ were in the lounge and one was in the head.
I guess my question, if I were heading the investigation, would be was ANYONE on the BRIDGE ?
That conflicts with the facts that the Captain and most of the crew were asleep, and it seems some of those 'on watch' were in the lounge.
Doesn't sound like there was any hi tech snooping going on.
Part of it has to do with the fact they are all leaving or heading to the same places.
Just got home...haven’t read it yet. Part I read so far was depressing.
Just a few observations:
1.) The official report from the USS Fitzgerald was made on a cell phone fifty minutes after the collision, as initial reports stated, and denied by the USN. I didn’t understand why they had to deny that, except perhaps it was embarrassing to admit the ship couldn’t communicate. I recall this was a sticking point from a lot of people until the time zone adjustment was made to the ACX Crystal track...but even then, some persisted, as the USN did.
2.) They didn’t call GQ until 13 minutes after the collision at 0143. I thought that was odd...I would think it would reflexively be the very first thing someone would do. Chaos.
3.) (no time indicated): Following the collision, FITZGERALD changed her lighting configuration at the mast to one red light over another red light, known as red over red, the international lighting scheme that indicates a ship that is not under command. Under international navigation rules of the road, this signifies that, due to an exceptional circumstance such as loss of propulsion or steering, a vessel is unable to maneuver as required. This was weird too. They didn’t call General Quarters, but someone did execute this detail. I wonder if it was a checklist, a retained fragment of process in someone’s head, or an automated process. It just seemed strange.
4.) 0540 FTZ reports second all-hands muster complete and seven Sailors are
unaccounted for. Seems like a long time, more than four hours to get an accurate muster, but...again, it was quite chaotic.
