There's a psychiatric disorder (I forget the term for it) where people are absolutely convinced that one of their arms or legs is an alien entity rather than a part of their bodies. Some indulge this psychosis by attempting to amputate their arms or legs themselves.
Nobody would think that such a person is fit for military service (or for much of anything else) because he would be an obvious risk and a liability. I fail to see any difference between the level of delusion between the guy who thinks that his arm doesn't belong to him and the guy who thinks that he's a girl in a man's body.

Body dysmorphic disorder. And you are right. People go to South America and pay $20,000 to have their arm or leg removed and then come back here and you go on disability. Disability isn’t the reason they do it but it is what they end up on
