I don't have time to watch stuff like that, but I can give you a very plausible explanation. The document was real, sent from Hawaii. The document could have been generated from bad data in Hawaii or generated by a bad actor. But the document arrived in the mail at the WH and stated what we see in the scan.
Once inside the WH a staffer scanned the document, then proceeded to copy and paste boxes and letters. Alternatively some OCR SW could have done that for them simply by scanning, recognizing boxes and letters, then replacing them in a layer with boxes and letters that result in artifacts like the matching boxes (1st and 2nd) in box 4 in the form. Those perfect matches cannot arise from a scan, but could arise from post processing. The simple reason for the post processing is to waste the enemy's time. We are the enemy, they are wasting your time.
