Regarding how long, here’s a NY Times item from late 2006 recognizing it as a new buzzword/buzz phrase. Which means it had been around for a bit before that.

Looking at some other posts here, 2006 does seem to be the start of reasonably widespread use of the term.

An excellent resource. It puts the lie to that "New America Foundation" graphic that SkyPilot posted on another thread, even as updated on their website (I checked) that shows less than 100 post-9/11 murders in the U.S. via Muslim terrorists.
