You are hugely mistaken. You are thinking only of the "cable news" networks: CNN, Fox News, and PMSNBC. Their numbers are still small compared to CBS-NBC-ABC, unfortunately.
While the "Big 3" of ABC, CBS, and NBC are not as influential as they once were, they still get around 6-9 million viewers for their nightly news (that is 20+ million per day for the 3 of them combined). Fox News gets under 3 million viewers for even its most popular shows.
To reach a lot of "middle America" a story has to break out of Fox News and out of Drudge, Trump twitter, etc. Those are a good start, but there is a lot of traditional media terrain out there which needs to be penetrated, against the wishes of the lying Mediascum.

And the MSM are very aware of that, very cynically smug about their ability to spike stories that don't "fit the narrative."
