Not surprised.
Normally they will not do surgery on people for elective issues unless all their numbers look good. Anasthesia problems are a lot more tricky and they just dont want anyone dying on a non critical operation.
Doing it for fiscal reasons if people can pay, that is another issue. In England their setups a bit different as they may not be able to get it done privately even if they can pay.

The reasoning is THEY CHOSE to do this to themselves.
Say:
WHY do we treat male AIDS patients..?
Over 1/3 of them have over 1,000 different sex partners.
I don’t hate them, but it seems like they are BEGGING for an expensive disease.
Insurance companies charge motorcycle riders more, as they pose a higher risk.
Smokers are charged more.
WHY must the public PAY ON BEHALF of a group that is DELIBERATELY contracting an expensive disease...?

In the US the CMS just has a different name for it. Medicare and Medicaid will pay Hospitals and providers based on the future health of the patient.
The future health of the patient depends on lifestyle choices over which the Hospital/Doctor have no control. The result will be that Hospital networks and physician networks will shift the patient load in order to maximize payment from CMS.
The providers are given incentive to treat those who are healthy and have healthy lifestyles and to not treat obese sexually promiscuous smokers.
