My own analysis is only anecdotal, but I only have seen this “look” twice before in my life and they were both from my dear elderly mom. The first time, she was suffering from a grand mal seizure. The second time, was during the last week of her life. As with DoughtyOne, I noticed this in him last summer and would speculate a neurological condition, for sure. Might be prescription psychotropic drugs, too.
Whatever it is, there is something up.
