They don't cut it off, they slice it down the middle and fold the two halves down to create the, uh, opening, and they re-purpose the sack skin to imitate the inner region. As I understand it, they end up with an open wound that has to be maintained ad nauseam lest it scar over.

I guess they could donate a portion of the testes. But who wants half a mangled ball sack that's used anyway?
