The problem here (it seems to me) is that the Baltimore PD had a well-known history of using paddy wagons to rough up arrestees (i.e. the "nickel rides").

He was faking a neck injury before he was put in the van. Most likely he got up and tried to give himself a real neck injury so he could blame it on the cops.
