Those doctors would be welcomed with open arms and a red carpet in almost any other country, including countries with actual free markets. So, what after that? An exit tax? Internal passports?

It’s really not that uncommon for insurers to do this as long as the application date fell within a no lapse of coverage period and you pay the premiums retroactively. The application date vs. the date all members on a policy are covered can take six weeks these days. You need certificates of birth and marriage license, certificates of credible coverage from previous insurer, and any COBRA letters qualifying you for an out of open enrollment period entry to the plan.
So all the customers of the uncooperative carriers dropped from the federal exchanges can't keep that insurance either? I somehow have the feeling that exchanges will be begging insurers to play in 2015.
So doctors are to treat people they aren't contracted to be paid for? Maybe the doctor should extract payment from the patient who could be reimbursed by Insurance/Obamacare. Yep that would work. Same goes for non-formulary drugs.

Surely they know that those providers OUTSIDE the network have not negotiated the prices and will charge the insurance companies a small fortune.
Thus, the insurance companies won’t do it. The providers KNOW the insurance companies won’t do it. Next up? Single payer!
