That wasn't a shot at the insurance company as much as it was against the doctor! He basically accused the doctor of pushing unnecessary surgery, a kind of fraud. Obama is always nasty towards doctors.
"I **like** nurses" Implying he hated doctors.
"Pediatricians steal the tonsils of children"
"Internists cut off the legs of diabetics"
Etc. He shows a petty resentment of doctors (you see this among a particularly stupid class of lawyers who secretly envy doctors their superiority) of physicians at every opportunity.
