Medical insurance is always getting more expensive because advances in medicine and medical care result in more complex and expensive treatments. How can a company like IBM make long-ranging decisions about retiree insurance coverage when nobody has any clue about what will constitute "standard medical care" in four or five years -- let alone in four or five decades?
