Your link is not directly to the pdf. This direct link is still up (at this time) and working:

http://www.irs.gov/pub/irs-tege/eotopici03.pdf

It is different from "eotopicf95.pdf" document linked on post-19. One uses the name "Carter Hull" & the other "Carter C. Hull."
