American sent out a letter yesterday to AAdvantage members, stating that almost all of the delays lately have been because pilots have been reporting maintenance problems at the last minute.
It requires them to hold the flight, and bring a mechanic to the gate to check it out. The mechanic says: "OK", and the flight departs about 30 minutes late. I had it happen to me this week.
