I don't know. Believe it or not the most strait laced people are Parkinson's patients. They do not smoke. They do not drink. They do not take drugs. They really the definition of obsessive compulsive at times which may be one of the early symptoms. It would be nice to see a couple hand writing samples of his over the last few years. For instance is his scratch getting smaller and smaller? Is there a tremor component? Does he keep long handwritten lists? These actually would be helpful.

The self-medicating with cocaine is not one that I have seen as a classic symptom. Smoking is another thing that is contraindicated. There are exceptions to every rule, however many neurologists will say the same things.

It is possible that he suffers from Parkinsonism or atypical Parkinson's. This can result years after a traumatic brain injury (TBI) or closed head injury. This is Parkinsonism and not true Parkinson's disease. I will discuss this with my wife. She is also an expert of atypical Parkinson's and may have not been looking for those symptoms.

Muhammad Ali suffers from this, IIRC.
