Sounds like a rotator cuff impingment, like baseball pitchers get. Had one once.

I was diagnosed with a frozen shoulder. After X-rays, 2CT scans and lots of therapy, nothing was helping. Took 4 yrs of my insistence on an MRI to get the proper diagnosis...torn rotator cuff!
