Maybe his left hand is bent like that to hide the joint he was smoking. The thumb and forefinger holding the butt of the joint with the other end toward the palm of his hand - cupped.
If his left hand were draped around a basketball then the left hand would have a more normal appearance, but underneath the left hand the white jersey is showing through. Odd!

Draw a horizontal line from the top of 0’s head to the guy on his RHS (our left.) Figure that kid is about 5-11”. Note 0’s head is about 2” taller. Note also legs,0’s knee is about 1.5” higher. That would mean that his upper leg is also 1.5” longer.
Do the math: 0 would be about 5 “ taller than that kid (again, the dark-haired kid, not the runt.)
They say that 0 is about 6’1.5” inches tall. He is taller in high school than he is now. Of course older people lose stature as they age and the discs in their back compress. But it’s hard to imagine he’d shrink 2.5”s.
The may have altered to make Obama look taller and more imposing. Or, they may have moved him in the center of the pic for the same reason. But there is definitely something weird about this photo.
By the way, it seems height has significance for journalists him; check this link from Salon,

This mixed review, so to speak, states that his height is 6-2”. Exaggeration? Most peg him at 6-1 to 6-1.5.
Maybe he wears lifts.
One last thought about the hands: their position suggests they were originally draped over a ball.
