I’m NOT a doctor, but I think he may have emphysema or a related COPD. He has the graying of skin and the blue lips, as well as the reported weight loss and noticeable fatigue.
His voice sounded a little rough at the State of the Obama address, also. Whatever it is, he is not well, and it seems that he is not getting enough oxygen. It could also explain (not excuse) his penchant for going on vacation and being outdoors in a warm environment (cold dry air and hot humid air can irritate the lungs).
