YOKOSUKA NAVAL BASE, Japan A piece of a Navy jet fell off in midflight Thursday afternoon, breaking a window at a residential home but causing no injuries.
The F/A-18E Super Hornet from Strike Fighter Squadron 27 lost a pump service handle at 2:15 p.m. while flying just northwest of Naval Air Facility Atsugi in Ayase City, base spokesman Tim McGough said Friday.
The handle was about 22 inches long and an inch in diameter, McGough said.
The Japanese governments South Kanto Defense Bureau and base officials assessed the damage last night but hadnt yet released any monetary damage figures Friday.
By Navy regulation, we do pre- and post-flight checks, and they are stringent, McGough said. We regret that this happened.
The Navy has opened an investigation into the incident, McGough said.
Heads will roll. Especially with the Japanese attitudes of late.
This would be like a car getting back on the road after a flat tire with the tire iron still on a wheel nut. Shouldn’t they account for the set of service tools as well as the state of the craft before leaving the ground?

I do not know Navy standards of practice. But, I imagine they are similar.

You have to safety wire in the handle after every use, and prior to every flight? Or is it only used when there isn't a start cart around?

Yamato City and Ayase City are built up right to Atsugi’s fence line. As soon as an aircraft crosses that line, anything that falls off is almost guaranteed to hit something of value.

Somewhere a first class petty officer is losing his "Quality Assurance/Safety Observer" (QASO) qualifications and will soon find himself the next night check supervisor in the line shack or possibly be placed in charge of first lieutenant buying snacks for the geedunk. That ain't gonna look good when he's up for chief.

Back in the day, when in Hong Kong, the junks etc would dart out in front of the liberty boats returning to ships, many a man was made rich by that manuever...
