I'm pretty sure that I've seen a long-form birth certificate, from Hawaii, from 1961, that a freeper posted to this site. They basically said, "Here's my form. I was born a week before Obama. Now -- where's his??"
I wish I had a link, but I don't.

Yeah, Hawaii didn't care about such things as which hospital or otherwise, and attending physician, etc. back then. (Do I need to post a sarc tag?)
