And how is California not at the bottom?
Not too many fatties in the Bay Area, Orange County, coastal San Diego County, or even western LA County. Much like New York, the fatties are concentrated in certain "ethnic" communities and the rural inland areas.

We've got things to do here, and were not sitting around the kitchen all day grazing...Ya know when it's constantly raining, snowing, or it's either 20 degrees or 105 and humid, people in those place tend to put on the feed bag and graze all day...Just the way it is.
why in the world would California be at the bottom? don't you realize how great the weather is here? we can be outdoors bike riding, skiing, hiking, swimming, running, walking, playing all kinds of sports... growing our own vegetables and fruit... eating fish... heck, we can go fishing in the ocean or in the lakes up in the mountains... which we do... and in many places in California, people care how they look--ergo, eat less, burn more calories... i would imagine that we also have a plethora of good doctors... since many doctors make good money, they can afford to live in California... we also offer much in the way of culture... being that i am in the central valley, i can go see a play just 45 minutes from where i live in any direction... Sacramento, San Jose, San Francisco...
