Ministry of Health has not established the exact diagnosis of the epidemic disease in the western regions of Ukraine. Health Minister Vasyl Knyazevich has given information about spread of diseases in the Ternopil, Ivano-Frankivsk and Lviv Regions today at the meeting of Cabinet of Ministers.
According to the Minister, the World Health Organization is ready to render assistance to Ukrainian experts and the Ministry of Health in order to establish the cause of death and development of disease flu in the Ternopil, Ivano-Frankivsk and Lviv Regions.
"We are working at receiving the vaccine against influenza A (H1N1) also", - said Vasily Knyazevich and asked the Ministry of Foreign Affairs to render assistance.
Answering question of Prime Minister Yulia Tymoshenko about the definition of the specific nature of the disease, the head of the Ministry of Health said: "The nature of infections is viral one. But at the moment it is not defined, is it the Californian influenza H1N1, or it is our seasonal influenza. To date, there is no precise diagnosis."

According to the Minister of Health, to date, 12 adults have died in the Ternopil Region. "There are problems, people of working age 20-45 years are sick, there is a problem with pregnant women (we have the death in the Ivano-Frankivsk and Ternopil Regions), 17 patients remain in the grave condition, 4 of them have artificial pulmonary ventilation", - said Vasily Knyazevych.

Ministry of Health reports that in Ukraine since the beginning of the epidemic of influenza, acute respiratory diseases and their complications (pneumonia, etc.) as of November 17 have died 328 people.
Relevant information is disclosed on the official website of the Ministry of Health.

Today, 18 November, Prime Minister Yulia Tymoshenko at the Cabinet meeting said the stabilization of the epidemic situation.
“We have a stable positive dynamics of Ukraine’s withdrawal from the influenza epidemic”, - said Tymoshenko. In particular, she noted that the daily number of cases went to the decline.
“Today we already have 56.5 thousand sick day, and for the first time the number of those who recovered exceeded the number of those who suffer from daily,” - added Tymoshenko. She believes that this dynamic is a consequence of the regional authorities, health professionals and government.
The prime minister also did not rule out that on 20 November in some regions of Ukraine will be lifted quarantine. “On Friday at a meeting of the State anti-epidemic commission, headed by First Vice Prime Minister Oleksandr Turchinov, to analyze all the assessments of the situation, which will give regional sanepidvrachi that we were able, based on their data, to assess the situation in each region to take all necessary decisions “, - said Tymoshenko.

However, today, November 18, deputy chief sanitary doctor of Ukraine Lyudmyla Muharskaya predicted that a second wave of influenza A/H1N1 in the country should be expected in December 2009 - January 2010, the third wave - the fall of 2010.

During a telephone conference dedicated to the situation in the country for the spread of influenza and ARI, First Deputy Minister of Health of Ukraine Vasyl Lazoryshynets focused analysis of disease among health care workers. As of November 16, 2009 ill 26 thousand health workers (2.4% of patients). The largest percentage of medical patients observed in the Chernivtsi region - 8.3%, Ternopil - 7.2%, Ivano-Frankivsk - 6.8%, Lviv - 5.8%. The percentage of hospitalized patients among the total number of health workers - 7%, while among the total number of patients - 5%. The largest number of health workers were injured in Kirovograd. The percentage of prescription with a diagnosis of influenza and pneumonia among health workers is 71%, while among the total population the figure is 31.3%, indicating a more severe course of disease is among health workers.
The alarming statistics contributed to the conference call to discuss the issue of compliance with anti regime in medical institutions and measures to protect health workers who provide medical care for influenza and ARI during the epidemic.
Chief Epidemiologist Ministry of Health of Ukraine Sergey lovingly reminded the representatives of regional regulations that govern these activities.
Health Minister of Ukraine Vasyl Knyazevich addressed to the regions and emphasized the personal responsibility of heads of medical institutions for the safety of health workers and their compliance with preventive regime. “Gentlemen, Chief Medical Officer, you have the relevant orders of the MoH, and your sacred duty to care for the safety of their employees. This applies to leaders of medical institutions at all levels. Each particular case of severe illness or death, medical workers will be carefully studied and doslidzhuvatysya the MOH and me personally “- said Vasily Knyazevich. The Minister of Health of Ukraine Vasily Knyazevich held telephone conference on fighting the epidemic of influenza and ARI in Ukraine.
As stated at the meeting last week, there was stabilization of the situation with the incidence of influenza and ARI in Ukraine as a whole and in the ten target cities. During the same period recorded more than 1 million 400 thousand cases.
Dynamics of the past week showed a slight slowdown in prevalence. For example, November 10, ill for one day about 90 thousand people, November 11 - about 70 thousand, November 12 - 61 thousand, November 13 - 55 thousand, 14 November - 38 thousand, November 15 - 17 thousand decrease the number of patients weekend experts explain the decrease in the number of traditional appeals to the clinics. The recent outbreak in Ukraine has raised concern that receptor binding domain changes are responsible. Although WHO has issued a new Ukraine situation update and has addressed preliminary changes in a news conference, they have not rule out receptor binding domain changes, and the recent announcement of investigations by two WHO Regional Centers (NIMR in Mill Hill, UK and CDC in Atlanta GA) suggests single nucleotide changes are under investigation.
One such change is the receptor binding domain polymorphism D225G which is present on recently released sequences from China and Australia. The HA sequence from China, A/Zheijiang/DTID-ZJU03/2009, is virtually identical to two earlier sequences from Yiwu (A/Zhejiang/DTID-ZJU02/2009 and A/Zhejiang-Yiwu/11/2009) and all were collected in September and appear to be from the same patient or contacts. The patient eventually recovered, but was seriously ill and hospitalized for several weeks. However, the three sequences from Yiwu match (see list here) sequences from an isolate, A/Hangzhou/1/2009, that is in another location in Zheijiang (see map), indicating D225G was appended onto the Hangzhou genetic background via recombination.
A sequence released Monday at GISAID by the WHO regional center in Australia, A/Sydney/2503/2009, also has D225G, but on a different genetic backbone. The 5’ end of the gene matches isolates from Singapore and Japan (see list), which do not have D225G. Thus, D225G is appended onto this background via recombination, but the background in Australia is distinct from the background in China.
Similarly, the two genetic backbones described above are distinct for two isolates in Sao Paulo, which were from fatal cases. The lung isolates signal the jumping of D225G from one genetic backbone to another. This concurrent acquisition has been described in H5N1, as well as the genetic hitchhiking of H274Y in seasonal H1N1.
This jumping of the same polymorphisms form one background to another signals major changes, especially when the polymorphism “in play” is a receptor binding domain change, which is cause for concern.
The recent activity in Ukraine raises concerns that similar changes are in play there, and the failure of WHO to release the sequences or comment on receptor binding domain changes significantly increases these concerns.

Mill Hill has released a series of sequences from patients in Ukraine. Four of the samples had the receptor binding domain change D225G. Three samples were from lung, one from a throat swab.

Mill Hill, a WHO regional center in London has placed sequences from 10 isolates from Ukraine on deposit at GISAID (see list below). They are to be commended for the prompt deposit of these important sequences. The availability of the sequences should put an end to wild speculation on the origins of the Ukraine outbreak.
All H and N sequences are typical for H1N1, as indicated in early WHO announcements. There are no large changes. Additional gene segments have been deposited from a subset of these isolates (but not analyzed below). There are silent changes that are in all or most Ukraine sequences, but the only HA polymorphism was the receptor binding domain change, D225G. This polymorphism was in the three lung, as well as the one throat sample. It was not in the nasopharyngeal washes or the isolate grown in MDCK cells suggesting the D225G may have a tissue tropism component and may allow for high levels of virus in the lung.
D225G was also found in necropsy lung tissue from fatal cases in Sao Paulo, further supporting tissue tropism associated with this polymorphism. The polymorphism has recently appeared on a series of different genetic backgrounds, supporting acquisition by recombination. The genetic backgrounds were geographically diverse. It was appended onto a genetic background specific for China as well as another distinct background found in Singapore and Japan. It has also recently appeared on backgrounds from Spain and Brazil. In addition, it was in isolates from last spring collected in the United States and Mexico.
The appearance of D225G on multiple recent genetic backgrounds raises concerns that the polymorphism is offering a selective advantage in association with multiple genetic backgrounds, and the selective detection of the polymorphism in lung and throat samples may indicate it is more widespread because of its absence from nasopharyngeal washes. Lung and throat sampling may be required for detection and determination of the true geograpohical reach of this change..
More information on outcomes for these patients, as well as results for lung and nasopharyngeal samples from the same patient, would be useful.
The prompt release of these sequences should help guide further analysis of the evolving swine H1N1.

The above tally is from the latest update from the Ukraine Ministry of health. The 344 dead represents an increase of 16 from yesterday’s total, which is similar to recent daily increases. The steady climb in fatal cases highlights the importance of the release of sequences by Mill Hill a WHO regional center in London.
Included in the sequences from 10 isolates were four HA sequences with the receptor binding domain change, D225G, which was found in the one throat and three lung samples. The change was not found in isolates from nasopharyngeal washes, suggesting D225G may lead to high concentrations of H1N1 in patient’s lungs. The high concentration of virus leads to a cytokine storm that destroys the lungs in a few days.
The finding of D225G in lung tissues raises concerns rergarding sequencing of isolates from nasopharyngeal swabs. The monitoring of this important genetic change in the receptor binding domain may require sampling of lung tissues or fluids.
The relationship between the negative nasopharyngeal washes and the lung samples would be useful.
Kudos to Mill Hill and WHO for the prompt release of the panel of H1N1 sequences from Ukraine patients.
