Actually that's very likely a fake. Note for example how the words "JOHN SIDNEY MCAIN III" *replace" rather than overwrite the line they are on.
McCain's mother, who shared the podium with him a few times during the campaign, maintains he was born on the Coco Solo Submarine Base Hospital. So does a an announcement in a local paper of the day, the Panama American, Monday August 31, 1936
It's on the seccond page, outlined in red. It's a bit like the Obama announcements, but gives the hospital name, and is a better image, "the Submarine Base Hospital".
Apparently McCain did show his BC to to at least one reporter, Michael Dobbs of the Washington Post. The reporter verified that the doctor named on the certificate, W.L. Irvine, was indeed stationed at Coco Solo in 1936. According to Dobbs, the BC shows the place of birth as the Coco Solo "Family Hospital". McCain does not show up in any of the Canal Zone birth
