I’m only familiar with the Los Angeles procedures. I never need any kind of card and they don’t check I.D., either. When I show up, they have me listed in two places—one by name, and one by street address (IIRC). Just tell them your name and address and then they ask you to sign to book next to your address.
Voila! If she forgot where her precinct is, she should be able to go to the County Website and put in her street address. It will tell her exactly where to go. If she is near the county Registrar’s office, she may want to drive down there and vote early.

If she didn’t do a change of address with the County, she may have a problem. I’d suggest calling the Registrar’s office and find out how to handle it. She’ll probably be showing up on the book for the old precinct instead of where she lives now.
Kerry won by 10% in 2004. These numbers are simply not believable.
I’m wondering if she can still vote in her old precinct, where she is registered, if she’s not in the hospital, that is.
When we lived in Cal., we moved right before an election, from one house to another, a block away. The voting place was the same. I went to vote (in a neighbor’s garage, and they wouldn’t let me vote because they knew that I had moved. My guess is that if I had been registered Democrat, there would have been no questions asked.

I found the answer for Shelby, TX—it is perfectly OK to vote at the old precinct.
She needs to call her particular County Registrar. Hate to see her end up being accused of vote fraud, or something.
Can I vote if I moved within my voting precinct?
Yes. If you are registered, you must report to your assigned polling place on Election Day and complete A Notice of Change of Address and make other changes, if applicable, and vote a regular ballot.
