I think you’re referring to the long-form COLB. The one online for BHO is a short-form one.

The short form document used in 1961 was a 2 1/2 inch by 3 1/2 inch green background wallet sized laminated document titled Birth Registration Card State of Hawaii Department of Heath Honolulu Hawaii. It has embossed State of Hawaii Seal stamped into and through the document and is signed by the Registrar General of the State of Hawaii. If one was trying to get a US Military security clearance, this document would not be accepted as proof of one's birth place. We tried. PS: The Brits finally got there exit papers. Paul made good pewter mugs and Sam made the tasty brew to put into the mugs.
