The short form document used in 1961 was a 2 1/2 inch by 3 1/2 inch green background wallet sized laminated document titled Birth Registration Card State of Hawaii Department of Heath Honolulu Hawaii. It has embossed State of Hawaii Seal stamped into and through the document and is signed by the Registrar General of the State of Hawaii.
