I don’t know if this helps but I have a birth certificate from the State of Hawaii. I will not post it because I won’t put that kind of private info out there but here are the details.
(by the way, it is my personal BC)

The year of birth is 1968.

The name of the document (on top): “Certificate of Live Birth”

Contains a stamped, raised, official Hawaii seal on the bottom.

Is Black in color....type is white.

Contains a File number on top right corner.

States in left top corner “State of Hawaii Department of Health Research and statistics Office”

Has the usual BC info, names of parents, address, ages, but DOES NOT ASK for Race of MOTHER nor Father nor Baby.

States change birth certificate forms. What you got in 1968 isn’t necessarily the same as the form used in 1961.

I pulled hubby’s copy of his birth cert.(USMC brat) but since it is October 1958 it states Territory of Hawaii. I am sure there were changes between 1958 and 1961, but on hubby’s it shows his father’s information first, and his mother’s second. It also includes occupations of the parents, and the title at the top “Certificate of Live Birth”. His is also black with white printing like it had been pulled from a microfiche.
