It will need to go up. As smokers die off or quit, they will need to replace the revenue that is being lost.
Therefore they will increase fees, which will drive more people to blackmarket or to quit, causing fees to go up to replace lost revenue.

How about a fee for having adultrous sex, it would help with the health care cost of STD's and other sexually transmitted disease.

A much higher fee will be required for same sex act because of the higher rate of spread of diseases including aids.

Of course, they would only do this if it is truly about health care costs.
